Concussion Requirements for California Youth Sports

The state of California has passed a new law surrounding concussions. As part of this law, parents of athletes under the age of 18 or athletes that are 18, must read and sign a concussion information sheet. Below is a link to the sheet. Once you have read the sheet, please check the box which is considered your electronic signature. If you do not complete this step, your child will not be able to participate as a swimmer at NHST.
